Responsibilities:
Clinical Liaison Sales LVN Responsibilities Meet with patients in the hospital or nursing facility to discuss services and transition to home on either home health or hospice servicesAct as a liaison for clinical appropriateness to our internal Branch Directors on the complexity of patients discharging and validating that the needs can be services at homeCoordinate home health services with physician as needed, which may include taking both written and verbal ordersPartner with Account Executives in the field to provide clinical education to referral sourcesReview and assist with hospital hold list, periodically participate in sales meetings and case conference
Qualifications:
Clinical Liaison Sales LVN Qualifications Associates or Bachelor's degree in nursing with LVN/LPN license1-3 years of experience Recommended Skills Clinical Works Health Care Home Care Hospice Nursing Passionate Estimated Salary: $20 to $28 per hour based on qualifications.
